WE WOULD NOW ASK THAT IF YOU ARE COLLECTING A PRESCRIPTION ON BEHALF OF ONE OF OUR PATIENTS, THAT INCLUDES A CONTROLLED DRUG,
YOU PROVIDE A LETTER OF AUTHORITY FROM THE PATIENT ALONG WITH A COPY OF ID
FOR BOTH THE PATIENT AND YOURSELF.
To sign up to use this service please come in to the surgery to obtain your password. You will need to bring photographic ID (e.g. passport or photo driving license) with you to sign up for this service. Once you have been accepted and allocated with a user name and password you can use the link below:

Access Your Records Online
Patients are now able to look at/print out their records relating to medications, allergies and adverse reactions. If you already have a password you need to contact the surgery to request access to your records.
PLEASE NOTE: Not all items are able to be sent by 'Electronic Prescribing' before nominating a chemist please check with them, that your items can be included.
• Many local pharmacies are happy for you to leave your list with them. They will deliver it to us, collect your prescription and dispense it ready for collection. Please allow longer time for this method.
Due to the increase in volume of prescriptions that we now receive, we regret to advise that our ‘prescription turn around’ time will increase from 48 to 72 hours. This will be with immediate effect. Please consider this when you are reviewing your medication needs and timing of your request.
In line with official advice and except in special circumstances, we do not take prescription requests over the telephone.
